Historic Clearance Extension Form

This is an extension form used for the project clearance that has been expired. An applicant of the project must write a letter to BCHP asking for an extension of Historic Clearance of a project that has been expired. The letter must include the Historic Clearance number and a reason why the project needs an extension.

mentioned above has been extended for one year from the date of Approved.
The project must follow all requirements and stipulations stated in the original historic clearance with a historic clearance number mentioned above that has expired including all of its attachments and all new additional conditions if applicable.
